You may review the Substitute Handbook to answer any questions you may have.

Please download the following PDF documents, either type in or print and write in your information, sign, and bring into the Office of Human Resources

• Social Security Form SSA1945
• Form I-9 Employment Verification
• W4 Federal Tax Witholding
• M4 Massachusetts Tax Witholding (if different from W4)
• OBRA Retirement enrollment form. More information at Massachusetts Retirement Board.
• Internal BPS Forms (for Substitute Teachers)
• Substitute Profile form for SubCentral
• A completed SubAssessment Certificate with a score of 85 or better OR a Massachusetts Educator License Certificate

Substitute Training

Complete the Boston Public Schools Substitute Teacher Training Program. A score of 85 or higher is needed to continue this process.

Other Documents

Schedule time with The Office of Human Resources Teacher Placement Team to bring in your completed forms and the following items:

1. Social Security Card (Info on Getting a Replacement SSN Card)
2. Another form of ID (passport or driver's license)
3. Voided check (if needed for Direct Deposit Forms)
4. Official transcripts for undergraduate and any graduate work completed
   
   1. Undergraduate transcripts must state that the degree was conferred and include degree conferral date.
   2. Candidates with educational backgrounds from abroad (excluding Puerto Rico, Guam, U.S. Virgin Islands or its territories) will need to contact the Center for Educational Documentation (617-522-4738), which provides professional assistance in evaluating the educational background of persons educated abroad. They can interpret from a number of languages (Spanish, Portuguese, German, Finnish, Swedish) for an additional $50. For direct translation, contact the International Documentation Center at 1-877-777-ASIA or www.eurasiacom.com.
      
5. Teacher License (only if you have one).  Information on Licensure is available in the Teachers Resources section under Licensure. We must see the original copy.
