What to Complete from Home

Social Security Card

Please find your original Social Security Card. We cannot hire you without reviewing this. If you have lost this card, see Info on Getting a Replacement SSN Card

Official Transcripts

We need the original transcripts from your college or university for any undergraduate or graduate work completed.  The transcript must state that an undergraduate degree was conferred, along with the conferral date.

Candidates with educational backgrounds from abroad (excluding Puerto Rico, Guam, U.S. Virgin Islands or its territories) will need to contact the Center for Educational Documentation which provides professional assistance in evaluating the educational background of persons educated abroad. Translation services are offered for an additional fee.

Direct transcript translation is available from the International Documentation Center

Massachusetts Teachers License

Information on Licensure is available under Licensure. We must see the original copy.  If you do not yet have a license in Massachusetts, see the Licensure page.

Previous Employment Letter

If applicable, this will verify previous full-time teaching experience. Full-time teaching experience only will be used for upgrades in salary.  Letters must:

  • Be an original document on official letterhead
  • Verify at least one hundred sixty (160) days under contract performed under a license granted by the Mass. Department of Education or certification that requires equal qualification. 
  • Include your dates of employment.

What to Bring With You to Orientation

Schedule time with The BPS Staffing Team to bring in your completed forms and the following items:

Please download the following PDF documents, either type in or print and write in your information, sign, and bring into the Office of Human Resources

And don't forget:

  • Ethics Reform Acknowledgment and Training Certificate
  • Social Security Card
  • Another form of ID (passport or driver's license)
  • Voided check (if needed for Direct Deposit Forms)
  • Official transcripts 
  • Massachusetts Teacher’s License
  • Previous Employment Letter

Dear BPS Human Resources:

Ana Garcia was employed by the Unified School District from September 1, 2004 until
June 30, 2007 as a full-time, fully-contracted, licensed teacher. Her attendance was
excellent and she reported for work no fewer than 160 days during each year of her
employment with us. Ms. Garcia has provided the attached copy of her teaching license
as proof of licensure during the period of her employment with us.
